Living with diabetes or pre-diabetes can be challenging and getting good information is crucial. Cornell Cooperative Extension of Suffolk County offers a wide array of services and educational programs for all residents to increase confidence in managing diabetes.
Our team of Registered Dietitians and Registered Nurses work alongside the healthcare teams at HRHCare health centers across Suffolk County.
Focusing on diabetes and pre diabetes, we provide one-to- one individualized education and care to help promote self-management of one’s health, wellness, and medical conditions. Topics covered include:
In addition to one- to- one sessions within the health centers, we offer a FREE comprehensive 6-week Diabetes Self- Management class series targeted to improving knowledge and confidence to any Suffolk County resident living with diabetes. These are offered throughout the year in locations across Suffolk.

In addition, community sites can request presentations on specific diabetes related topics.
