Position Summary:

As a member of the Diabetes Self-management Education (DSME) program team, provide Spanish language interpretation and outreach support to Diabetes Educators. Foster and maintain a working relationship with health center staff, community agencies and local organizations to make referrals of individuals for additional health services. Observe outreach and networking approaches to maximize impact at assigned clinic. Represent the association and the Family Health & Wellness Program to the general public, community leaders, government officials and Cornell University.

Works collaboratively with diabetes educators within the Association and community. Employee will normally work at assigned clinic in group setting with daily supervision and monitoring.

Required Qualifications:

  • Associate's Degree and 1 year volunteer or related work experience; or High School Diploma and 3 years volunteer or work experience.Bilingual; fluent in English and Spanish
  • Ability to meet travel requirements associated with this position.
  • Meet acceptable background check standard, if required.
  • Ability to work evenings and weekends as essential job functions require

Preferred Qualifications:

  • College level course work in nutrition
  • General knowledge of diabetes prevention and control and ability to teach this information
  • An understanding of the needs of limited-income families and communities
  • Experience and/or interest in working with programs that serve diverse communities
  • Ability to handle confidential information appropriately
  • Good writing, speaking and organizational skills
  • Ability to set priorities and meet deadlines
  • Ability to work independently within program and agency guidelines
  • Ability to work well with a variety of people and provide a role model for participants
  • Ability to transport self and equipment as needed
  • Ability to accept direction and supervision
  • Ability to work as a team member
  • Computer literacy; competent in MS Word, Excel

Cornell Cooperative Extension of Suffolk County is on the forefront of Diabetes Self-Management education. This collaborative program with the Suffolk County Department of Health reaches thousands of residents yearly.

We are currently looking for a part time bilingual outreach worker to assist program’s RN’s & RD’s with translating patient education and helping to create a culturally relevant plan of care. Must be comfortable working with diverse communities and understand importance of cultural competency and health literacy.

Essential Responsibilities include:

  • Translating patient education
  • Data reporting
  • Collaborating with the healthcare team to help individualize the patient’s plan of care.
  • Attending monthly team building meetings
  • Traveling to community sites
  • Some evenings and weekends as needed by program.
