Position Summary:
Serve as a Community Education Team(Comm-ED) Program member to assist with implementing, and evaluating the Diabetes Education Program. Educator will be responsible for promoting program objectives in Community Ed areas, emphasizing diabetes education and diabetes self-management in both Health Center and community settings in Suffolk County. The main objective is to provide prevention-focused and research-informed diabetes education to meet Suffolk County residents and professionals' diverse needs. The position will follow research-based information from the ADA Clinical Practice Recommendations and the National Standards for Educational Programs to enhance the diabetes self-management practices of those who live with or work with pre-diabetes or diabetes to optimize health outcomes. This individual represents Cornell Cooperative Extension's to the public, community leaders, government officials, and Cornell University as part of program delivery. This position is funded by a contract with the Suffolk County Department of Health Services.

Salary: Non-exempt $20.00 (per hour)
Hour: Part-time 24 hours (3 days a week)
