Association Subject Educator II – Diabetes Educator - RD
Serve as a member of the Family Health and Wellness (FHW) Program to assist with, development, implementation and evaluation of the Diabetes Education Program. Educator will be responsible for promoting program objectives in areas of FHW with an emphasis on diabetes education and diabetes self management in both clinical and community settings in Suffolk County. The main objective is to provide prevention focused and research informed diabetes education to meet the diverse needs of Suffolk County residents and professionals.The position is responsible for using current research based information from the ADA Clinical Practice Recommendations and the National Standards for Educational Programs to enhance the diabetes self management practices of those who live with or work with prediabetes or diabetes in order to optimize health outcomes. This individual represents Cornell Cooperative Extension’s family health and wellness programs to the public, community leaders, government officials and Cornell University as part of program delivery. This position is funded by a contract with the Suffolk County Department of Health Services.
Salary Range: $24.00 per Hour
