Position Summary:

The Diabetes Educator position provides program delivery for diabetes education and diabetes self-management in clinical and community settings in Suffolk County as part of the Community Education Program and under the Community Ed Issue Leader's supervision. The main objective is to provide prevention-focused and research-informed diabetes education to meet Suffolk County residents' and professionals' diverse needs.The position is responsible for using current and established research-based information from the ADA Clinical Practice Recommendations and the National Standards for Educational Programs to enhance the diabetes self-management practices of those who live with or work with pre-diabetes or diabetes optimize health outcomes. This individual represents Cornell Cooperative Extension's family health and wellness programs to the public, community leaders, government officials, and Cornell University as part of program delivery.This position is funded by a contract with the Suffolk County Department of Health Services.

Required Qualifications:

  • Bachelors or Equivalent Education. Associate Degree and 2 years transferrable program/function experience are acceptable.
  • Degree in Dietetics (RD).
  • Experience in diabetes education, community outreach, and health care.
  • Ability to meet travel requirements associated with this position.
  • Ability to work flexible hours, which may include evenings and weekends, as appropriate.
  • Ability to communicate effectively through oral, written, and visual channels using traditional tools and methods and electronic technology (e.g., video, audio, computers, etc.).
  • Demonstrated strong customer service skills.
  • Detail-oriented.
  • Ability to follow procedures and written instructions.
  • Must demonstrate a high degree of integrity, sound professional judgment, and the ability to handle confidential and sensitive information.
  • Proficient with windows-based software, including the use of the internet and Microsoft Office Suite, including Outlook, Word, PowerPoint, Publisher, and Excel.

Preferred Qualifications:

  • Ability to effectively participate in professional team efforts.
  • Ability and willingness to work with diverse audiences and maintain cultural sensitivity.
  • Ability to plan and organize personal work responsibilities according to priorities developed with the immediate supervisor.
  • Experience with Cooperative Extension or other similar educational organizations.
  • Ability to relate effectively to co-workers, advisors, community, and professional leaders.
  • Ability to identify and utilize program resources from the organization and the community.
  • CDE (Certified Diabetes Educator) credential or can obtain within the first three years of employment.

Salary: Non-exempt $24.00 (per hour)

Hour: Part-time 24 hours (3 days a week)
