Position Summary:

Serve as a member of the Family Health and Wellness (FHW) Program to assist with, development, implementation and evaluation of the Diabetes Education Program. Educator will be responsible for promoting program objectives in areas of FHW with an emphasis on diabetes education and diabetes self management in both clinical and community settings in Suffolk County. The main objective is to provide prevention focused and research informed diabetes education to meet the diverse needs of Suffolk County residents and professionals. The position is responsible for using current research based information from the ADA Clinical Practice Recommendations and the National Standards for Educational Programs to enhance the diabetes self management practices of those who live with or work with prediabetes or diabetes in order to optimize health outcomes. This individual represents Cornell Cooperative Extension’s family health and wellness programs to the public, community leaders, government officials and Cornell University as part of program delivery. This position is funded by a contract with the Suffolk County Department of Health Services.

Required Qualifications:

  • Master’s Degree with experience relevant to the position. Bachelor’s Degree plus 4 years transferrable relevant evidence based research experience may substitute.
  • Degree in Nursing (RN).
  • Experience in diabetes education, community outreach, and health care.
  • Ability to communicate effectively through oral, written, and visual channels using traditional tools and methods, as well as electronic technology (e.g., video, audio, computers, etc.).
  • Demonstrated strong customer service skills.
  • Detail oriented.
  • Ability to follow procedures and written instructions.
  • Must demonstrate a high degree of integrity, sound professional judgment and the ability to handle confidential and sensitive information.
  • Proficiency with windows based software including use of the internet and Microsoft Office Suite including Outlook, Word, PowerPoint, Publisher, and Excel.
  • Ability to meet travel requirements associated with this position.
  • Ability to meet acceptable background check standards, if required.
  • Ability to work flexible hours which may include evenings and/or weekends, as appropriate.
  • Ability to effectively participate in professional team efforts.
  • Ability and willingness to work with diverse audiences and maintain cultural sensitivity.
  • Ability to plan and organize personal work responsibilities according to priorities developed with the immediate supervisor.
  • Ability to relate effectively to coworkers, advisors, community and professional leaders.
  • Ability to identify and utilize program resources from the organization and the community.

Preferred Qualifications:

  • CDE (Certified Diabetes Educator) credential.
  • Experience with Cooperative Extension or other similar educational organizations.

Salary Range: $27.00 per Hour

Cornell Cooperative Extension of Suffolk County is on the forefront of Diabetes Self Management education. This collaborative program with the Suffolk County Department of Health reaches thousands of residents yearly.

Currently seeking a part time RN to serve as diabetes educator in a federally qualified health center as well as in the community setting. Must be comfortable working with diverse communities and understand importance of cultural competency and health literacy.

Essential Responsibilities include:

  • Teaching Diabetes self-management skills to patient’s one on one.
  • Working with the healthcare team to help individualize the patient’s plan of care.
  • Teaching Diabetes self-management courses in the community.
  • Keeping current with evidence based practice and standards of care.
  • Participate in various PI/QA projects and data reporting.
  • Collaborating with other CBO’s for public health initiatives.
  • Attending monthly team building meetings
  • Precepting student intern and medical residents.
  • Some evenings and weekends as needed by program.
